What this site collects

The contact form asks for your name, your practice, your email address, optionally your phone number, your state or ZIP code, what equipment you are asking about, and whatever you write in the message box. Submitting it also sends technical details that come with any web request (your IP address, your browser's user-agent string and the page you submitted from) to the service that delivers the form. There is no analytics script, no advertising pixel and no third-party tracker on this site.

Where it goes

Form submissions are processed by Web3Forms, which delivers your message to my email. Web3Forms may retain and process what you send as described in its privacy policy (currently for up to three years) and may pass your IP and email address to the spam-prevention providers it uses. Once the message reaches me it sits in my inbox like any other email.

What it is used for

Replying to you. Your details are not added to a mailing list, are not sold, and are not shared with the manufacturers whose equipment is listed on this site unless you ask for that, for example if arranging a demonstration requires it, which will be said at the time.

Cookies

This site does not set cookies.

Removing your information

Call or text 612-845-6511, or ask through the contact form, and I will delete the correspondence from my own records. Web3Forms holds its own copy on its own schedule, so I will also ask them to delete it, but that part is theirs to action rather than mine to promise.

Clinical and billing information

Please do not send patient information through this form. It is a sales enquiry form, not a clinical channel, and it is not built for protected health information.
